As healthcare becomes more value-based, interest is growing in approaches that address both medical needs and social determinants of health (SDOH). Even patients now recognize the critical role that SDOH plays in determining their health status, with a recent study showing that 80% agree that access to stable housing, nutritious food, and reliable transportation has an impact on their ability to access care and be healthy.

Many organizations are finding that partnering with community-based organizations (CBOs) is the most cost-efficient and effective strategy for advancing SDOH initiatives. There is growing awareness that partnering with CBOs is critical to moving from basic screenings for social needs to comprehensive interventions that address the full spectrum of patients’ medical and social issues. As a result, CBOs have new opportunities to emerge as leaders in building programs that successfully address SDOH. These new partnerships, however, can raise challenges for traditional healthcare entities.

How can payers, providers, and CBOs forge partnerships that align with their missions, support quality care and services, and leverage sustainable funding solutions? Manatt Health, Healthier Here and Montefiore Health System come together to reveal the answer in a new webinar. Click here to register free. The session will:

  • Explain how traditional healthcare entities are developing partnerships with CBOs to pursue holistic, integrated approaches to population health.
  • Discuss the advantages of partnering with CBOs to create comprehensive solutions for addressing SDOH needs, particularly for high-needs, high-risk populations.
  • Share approaches for overcoming the barriers to developing successful partnerships between traditional healthcare organizations and social services organizations, including contracting, data exchange, and payment approaches and sources.
  • Present proven strategies for supporting successful and sustainable partnerships among payers, providers, and CBOs.
